Oriental Medicine Diagnosis and Treatment for Low Back Pain

Low back pain is one of the most common ailments experienced by modern people. While Western medicine mainly focuses on structural issues like lumbar discs, joints, and muscle strain, Korean Medicine (KM) evaluates both structural components and internal imbalances, such as the weakness of organ systems, blood stasis, and phlegm-fluid retention.

1. Diagnostic Types of Low Back Pain in Korean Medicine

In traditional Korean medical texts like Dongui Bogam, back pain is categorized based on its root cause and characteristics. The main diagnostic types include:

Kidney Deficiency Back Pain (Shen Deficiency): Caused by aging, overwork, or chronic ailments. It presents as a dull, lingering pain accompanied by weakness or a cold sensation in the knees and lower back.

Blood Stasis Back Pain (External Trauma/Stagnation): Caused by physical trauma, accidents, or poor posture over time. The pain is sharp, stabbing, fixed in a specific spot, and tends to worsen at night.

Cold-Dampness Back Pain: Caused by exposure to cold and humid environments. The lower back feels heavy, stiff, and cold, with symptoms worsening on rainy or cold days, and improving with heat application.

Phlegm-Fluid Back Pain: Occurs when digestive dysfunction leads to the accumulation of metabolic waste (phlegm-fluid). Pain often radiates or moves around the back and shoulders with a sense of heaviness.

Qi-Stagnation Back Pain: Triggered by emotional stress or mental tension. It is characterized by tightness in the lower back that fluctuates with stress levels.

2. Integrative Korean Medicine Treatments

Korean medicine aims not only to relieve immediate pain but also to restore bodily balance, strengthen surrounding tissues, and prevent recurrence.

Acupuncture & Pharmacopuncture: Acupuncture targets key acupoints (such as BL23, BL40, GB34) to relieve muscle tension and stimulate circulation. Pharmacopuncture injects purified herbal extracts into acupoints to reduce inflammation and accelerate tissue recovery.

Chuna Manual Therapy: A hands-on spinal manipulation technique performed by Korean Medicine doctors. It realigns misaligned spinal joints and pelvis, reduces pressure on discs, and restores structural balance.

Herbal Medicine: Customized herbal formulas are prescribed according to individual diagnostic types—replenishing kidney energy for deficiency, clearing stasis for traumatic pain, or warming the channels to expel cold-dampness.

Cupping & Moxibustion: Cupping uses negative pressure to improve local blood flow and release blood stasis. Moxibustion applies thermal therapy to warm the meridians, relax tight muscles, and promote healing.

Low back pain is often a signal of deeper bodily imbalance. A comprehensive diagnosis addressing the root cause is the key to achieving long-term recovery and preventing chronic discomfort.
