Back Pain?

“Sudden Back Pain? Low Back Pain in Traditional Korean Medicine: Causes and Treatments”

Sudden low back pain can severely disrupt daily life. In Traditional Korean Medicine (TKM), back pain is one of the most common complaints among patients. Rather than viewing back pain merely as a localized muscle issue, TKM diagnoses and treats it based on internal energy flows (Qi and Blood) and organ balance.

1. Primary Causes of Low Back Pain in TKM

According to classic medical texts like Dongui Bogam, back pain is divided into various categories depending on its root cause:

  • Kidney Deficiency Back Pain (Shen Deficiency): In TKM, the ‘Kidney’ system governs bones, the spine, and vital energy. Overwork, aging, or stress weakens this energy, causing a dull, chronic ache accompanied by weakness in the legs.

  • Blood Stasis Back Pain (O-hyeol): Caused by acute trauma or poor posture that damages microvessels, leading to stagnant blood. Characterized by sharp, stabbing pain that often worsens at night.

  • Phlegm-Fluid Back Pain (Dam-eum): Stagnation of metabolic waste fluids blocks the meridians around the waist, causing a heavy, stiff sensation in the lower back.

  • Wind-Cold-Dampness Back Pain: Caused by exposure to cold or humid environments, which tightens muscles and joints. Pain typically worsens on rainy or cold days.

  • Acute Lumbar Sprain (Jwa-seom): Sudden strain caused by lifting heavy objects or twisting the waist awkwardly.

2. TKM Treatment Approaches

TKM treatments focus not only on relieving pain but also on strengthening surrounding muscles and restoring natural balance:

  • Acupuncture & Pharmacopuncture: Stimulates key acupoints (such as Shenshu BL23, Weizhong BL40) to unblock meridian flow. Herbal extracts are directly injected into inflamed tissues to speed recovery.

  • Chuna Manual Therapy: A hands-on spinal manipulation technique performed by TKM doctors to realign the spine and pelvis while releasing muscular tension.

  • Herbal Medicine: Prescribed to eliminate stagnant blood, reduce inflammation, and nourish the bones, ligaments, and discs to prevent recurrence.

  • Cupping & Moxibustion: Cupping improves local microcirculation by releasing stasis, while moxibustion applies therapeutic heat to warm the meridians and ease stiffness.
